To register for the Swing for the Kids Golf Tournament, please fill out the following form. If you are registering as an individual, please just fill out the first part.

Team Payment Options (Team Registration Only) You can either make one payment, or pay for different members seperately. If "One Payment" is selected, there will be a single payment of $320 via PayPal or Check. If Seperate Payments is selected, then multiple payments can be made in differnt amounts. For example: One payment by PayPal of $160, another payment by PayPal of $80, another payment by Check of $80. More information on how to make payments available after registraiton.
